| The Doctoral
Committee Appointment Form officially establishes the doctoral
committee; the signatures of the committee members indicate their
willingness to serve on the committee. Once filed, this form becomes
a permanent record and remains in effect unless a new Doctoral Committee
Appointment form is submitted indicating the removal and addition
of committee members. The only signatures needed for this revision
would be the commitee members being removed and added, plus the
Department Head.
