Please exclude my directory information from publication. I understand that this action will result in my directory information being excluded from:

  • The printed Student Directory, published each Fall term,
  • UT's Online "People Search" Directory,
  • The Commencement Program the term I graduate,
  • The online list of confirmed degrees, and
  • Data provided to non-University (external) entities. 

Directory information includes name, local, home & email addresses, telephone listing, birth date, participation in officially recognized activities and sports, weight and height of athletic team members, dates of attendance, degrees and awards received, major field of study, classification, college, the most recent previous educational agency or institution attended by the student.

I understand that the printed Student Directory is compiled following the fourteenth calendar day from the beginning of Fall term classes, and that I must submit my request for exclusion from that directory prior to that date. I further understand that there is no deadline for exclusion from the online "People Search" directory.

This action will remain in effect until I ask that it be revoked.
